Please fill out the following application if you are signing up for a HFH short term trip. This form includes general contact info, a spiritual assessment, a trip covenant, and liability and risk releases. Please answer questions honestly and to the best of your ability. If you are under 18, a parent or guardian must also sign this form.

Welcome Packet

This welcome packet provides all the information you should need after signing up for a trip.

Medical Form

All trip applicants are required to fill out and provide us with specific medical information so we can make sure your needs are met on the trip. Download the form and fill out digitally, then email it to info@hopeforhispaniola.org. You can also print it out and fill it out manually.

Packing List

We want to make sure you're adequately prepared for the trip, from dressing for a different culture and environment to having the right documents and medication. Please carefully review this packing list while you prepare!

Missions Trip FAQ

Who can go on a missions trip with Hope for Hispaniola?

We welcome everyone on our trips! We strive to make them family friendly and open to all walks of life. Many children and teens join our trips and are valuable contributors, boldly sharing their faith. For more information if HFH trips are right for you, contact Jake Vincik.

Do I need to get vaccines for the trip?

We strongly encourage you to consult your physician on what medical precautions you should take. However, taking precautions against malaria should be a top priority.

Where do missions trip teams stay?

Teams stay at our Missions Center located near Higuey. The Missions Center has men's and women's dorms, apartments, and full facilities.

What should I pack for a trip?

We recommend bringing sunscreen, bug spray, comfortable closed-toe shoes, a hat, toiletries, snacks, and culturally-appropriate clothing. Please consult your trip's intro packet to ensure you respect the culture of the people you're serving.
