Please fill out the following application if you are signing up for a HFH short term trip. This form includes general contact info, a spiritual assessment, a trip covenant, and liability and risk releases. Please answer questions honestly and to the best of your ability. If you are under 18, a parent or guardian must also sign this form.

Our Mission

To Participate in the glorious redeeming work of God that he initiated through Jesus Christ our Savior and continues by his Holy Spirit. Hope for Hispaniola strives to participate in the ongoing work of God by encouraging and strengthening local churches in the Dominican Republic through evangelism, discipleship, and service within the community.

Trip Covenant

While glorifying the name of Jesus, further spreading of the Gospel and supporting the local church in the Dominican Republic, I agree to abide by the following:

1. I shall abstain from any behavior or practice that does not honor our Lord Jesus Christ.
2. I shall follow the decisions of those who are designated leaders while on this trip. I will not leave my assigned area of ministry or separate myself from my assigned group without first obtaining permission from a team or group leader.
3. I shall immerse myself in the local culture as much as possible and joyfully affirm the standards of the local body of believers, including areas of dress code.
   1. No tank tops, sports bras, skin tight pants/shorts such as spandex, visible underwear, shorts or dresses shorter than the extended hand, or clothing with inappropriate slogans or advertisements.
   2. Other dress requirements to be discussed at the group meetings.
4. I shall refrain from making derogatory comments about local cultures and congregations, complaining, gossiping, criticizing, and using obscene or insensitive language or humor.
5. I shall refrain from arguments regarding people, politics, sports, religion, race, or traditions.
6. I shall refrain from using tobacco products or illegal drugs and from consuming alcohol.
7. I shall develop and maintain a submissive and cooperative attitude towards my teammates and others we encounter and be as flexible as possible as plans will change from day to day.
8. I will respect my fellow participants' right to privacy, possession and "peace and quiet".
9. I shall send out support letters to ask for prayer and to raise funds for the whole trip, including the building projects.
10. I acknowledge that participating in a mission trip can present numerous unexpected and undesired circumstances, but the rewards of conquering such circumstances are immense.

Risk Statement

This Risk Statement is to advise participants of the potential risks involved in living and working in third world developing nations and to realize and take full responsibility for the consequences as one assumes those risks. Living and working in other cultures carries with it certain risks not found or associated with work in an industrialized nation such as the United States. These risks can include hazards to both your person and property through accidents, disease, weather, etc. as well as inadequate medical services and supplies. We realize that it is not possible for us to predict, or fully prepare you for every circumstance you will face.

However, it is our goal to advise all members of Hope for Hispaniola missions of the assumed risks associated with mission work in a foreign country and Hope for Hispaniola's policies, through this Risk Statement.

In the event of an emergency that requires medical services while on the field, all reasonable efforts will be made to provide the necessary service. If an evacuation is needed for more specialized care than is available in the designated area of ministry, that will be coordinated with and under the direction of the medical/evacuation insurance provider that is required for your participation in the Hope for Hispaniola mission trip. You are urged to adhere to the list of immunizations and vaccinations before you travel to the country of ministry.

Liability and Waiver Release

In consideration of me or my minor aged child being allowed to participate in the trip sponsored by Hope for Hispaniola, its partners and/or agents and in consideration of the benefits to be derived there from, I or my minor aged child hereby release Hope for Hispaniola, its partners and/or agents and their present and former elders, pastors, directors, employees, agents and their heirs, administrators, executors, successors, and assigns from all claims and liabilities of any kind, whether known or unknown, which arise from or are connected in any way with my participation or the participation of any member of my family including my spouse or minor child, in a Hope for Hispaniola sponsored mission trip. I recognize that the conditions in some of the places to which I, or my minor child, will travel may not be of the same standard as the conditions to which I, or my minor aged child, is accustomed. I, or my minor child, further realize that there are certain health risks as well as other risks to personnel and property, and I enter into participation in this trip and agree to the participation of my spouse or minor child with knowledge of those risks. If for any reason I, or my child, is unable to complete the planned stay on the mission trip, I assume full responsibility for all expenses incurred for my return home.

In the event of an emergency, I hereby authorize a leader of this trip, as an agent for me or my spouse or my child to consent to: any x-ray examination; medical, dental or surgical diagnosis; anesthesia; treatments; hospital care advised and supervised by a physician, surgeon or dentist (as appropriate). I expect my family to be contacted as soon as possible.

This is only for emergency situations should the individual be incapable of making rational decisions, or is a minor whose parents cannot be immediately reached. In any situation, every effort will be made immediately to reach the person to contact listed on the application.

Signatures below will indicate the following:

• All the information have provided in this application is true to the best of my knowledge.
• have read the Hope for Hispaniola mission statement and agree to respect it in all regards and not propagate convictions not contained therein.
• Having read the Risk Statement, I am aware of the hazards and risks to my person and property associated with serving overseas in a missions capacity.
• This is to certify that I will not hold Hope for Hispaniola liable for injury, disease, or delay of return, or any other claims, while undertaking a Short Term Mission trip with Hope for Hispaniola.
• I bear the full responsibility for all costs incurred on my behalf, even if I withdraw from the team at a later date.
• understand that donated funds sent in to Hope for Hispaniola for the trip are not refundable. All the information I have provided in this application is true to the best of my knowledge.

Media Release

The undersigned enters into this Agreement with Hope for Hispaniola.

I have been informed and understand that Hope for Hispaniola will be recording the EVENT and that my name, likeness, image, voice, appearance and/or performance are being recorded.

I grant Hope for Hispaniola, or its designee, the right to use my name, likeness, image, voice, appearance, materials and performance as recorded on or transferred to videotape, film, slides, photographs, audio tapes, or other media, now known or later developed. Hope for Hispaniola, or its designee, shall have complete ownership of the Product in which I appear, including copyright interests, and I acknowledge that I have no interest or ownership in the Product or its copyright. I also grant Hope for Hispaniola and its designees the right to broadcast, exhibit and otherwise distribute the Product, either in whole or in part.

I have read the foregoing and understand its terms and stipulations and agree to all of them.
